Prepare Donation Agreement

• The agency and donor will prepare a Donation Agreement (DA) detailing tasks and responsibilities. To view the Donation Agreement, click here.

Obtain Agency/Donor/OCTO Approval

• Once the roles, responsibilities, scope, and deliverables of the donation are established, the agency head and a representative of the donor firm will execute and sign the Donation Agreement.
• The agency head will submit the Donation Agreement to OCTO for approval and signature.

Obtain Mayoral Approval

• OCTO will submit the Donation Agreement to the Executive Office of the Mayor for approval.
• The Mayor, or his designee, has final approval authority of the Donation Agreement.

Provide Donation Agreement Services

• Upon approval from the Mayor or his designee, the donor firm will provide services to the agency. The agency and the donor firm will hold regular project reviews to gauge progress and restructure activities as needed. The Donation Agreement must contain performance criteria for the work done.
• All work must be completed in a timely manner and be of the same quality as paid work. Both the agency and the donor will be completely responsible for the performance of their respective and mutual roles.

Close Out Donation Agreement

• The agency and the donor firm will hold a meeting at the conclusion of the project. Donor firms are expected to complete all obligations under the Donation Agreement. Failure to complete the requirements will preclude the organization from future participation in the District's Adopt an Agency Program.
• Organizations that successfully fulfill their obligations will be encouraged to enter into additional Donation Agreements.
